I am needing a logo / avatar graphic for a site that will be used as a online tool to help people manage their diabetes using goals to track diet and exercise. There will also be a nutrition tool as part of the site.

I am needing a logo / avatar graphic for a site that will be used as a online tool to help people manage their diabetes using goals to track diet and exercise. There will also be a nutrition tool as part of the site. There will be a follow up contest for web site style css work for the site once the logo is chosen. I need the output in the form of a .psd file from photoshop.
Header graphic that has the logo and "diabook" in 3d type text. Color scheme is open. what ever works the best will be used. The theme is using meters to track goals for diet, nutrition, and exercise. The name is Diabook.
