This application is currently in development and has several screens which can be seen in the attached video. For this contest the only screen that needs to be designed is the filling out of the checklist. This is the screen in the attached images.
I forgot to mention something about the tab bar in the original design brief. The way the app works is that the user would fill everything out on the HEADER tab. Then answer everything on the QUESTIONS tab. Then they would go to the REVIEW tab where the would submit their answers. I want the tab bar to somehow represent that there is order to the tabs. I don't think the default iOS tab bar works well to represent the flow of the app. I would like the designs to implement a tab bar (or some other control) that represents the flow described above. Also once the user fills out all the information on the HEADER tab or answers all of the items on the QUESTIONS tab, I would like those tabs to somehow represent that it has been completed.
This update is not changing the original design description. It is additional information.
SCREEN THAT NEEDS DESIGN - All attachments are showing what I will be referring to as the "Question Screen". This screen can show hundreds of different sets of questions. Depending on the questions, this screen will look different. However, it will always be comprised of a fixed set of UI elements. You can have 1 design that shows all of the UI elements. You can have multiple designs, where each shows different UI elements.
The following is the list of UI elements that the make up the checklist screen. These elements are identified in the Checklist Breakdown attachments.
CHECKLIST INSTRUCTIONS - These are optional instructions that relate to a specific checklist. Most checklists won't contain these instructions.
HIDE ANSWERED QUESTIONS SWITCH - This switch will hide any checklist questions that the user answered. In my current design there is nothing that explains what the switch does. This will need to be added to the design.
SUPPLEMENTAL TEXT - Some checklists will have additional supplemental text for the questions. The user can decide to show supplemental text for all questions or none at all. They cannot show supplemental text for only specific questions. This is by design.
SECTION HEADER - In addition to displaying the title of a question, it can also be clicked to show supplement text for all of the questions. If you can find a better way to show/hide supplemental question text, please add that to your design.
CHECKLIST QUESTION NUMBER - All checklists questions have numbers. These are always numeric and increment by 1. They currently have two different styles. Style 1 is the default. Style 2 indicates that the question contains notes.
CHECKLIST NOTES - For each checklist question, the user can choose to add additional notes. Than can also add photos. Once the user adds the notes, they can choose to hide them. This is why two styles of checklist numbers was needed. If you can find a better way to indicate that a question contains notes, feel free to include this in your design.
TALLY CONTROL - These include two numbers that can be incremented to show the counts of items in the checklist question. In most checklists, these numbers will represent the same thing for all questions. For example, in the screenshot that has a checklist title of "PBS HPI Observation", the left number represents SAFE while the right number represents AT RISK. The design will need to figure out a way to label each one of these numbers.
SEGMENTED CONTROL - In the screenshots all the answers highlight the current answer with a blue square. This will need to be changed in the design. There can be two variations. In variation 1, all items will have the same selected look. In variation 2, each answer will have a different color when selected. For example YES/GREEN, NO/RED, NA/BLUE. They only need to be this color when the answer is selected.
FOLLOW UP QUESTION - Depending on the answer chosen by the user, a followup question can get inserted below the current question
